Universal Lifeline Telephone Service (ULTS)
Lifeline local phone service is available to low-income consumers who qualify.
Steps to Enroll
To begin receiving Lifeline program discounts, you will need to submit a Lifeline Telephone Application and proof of participation / eligibility requirements to Cox.

IOWA RESIDENTS CONTACT COX at 855-684-1029 to have an application mailed to you.
Once Cox has received these, one of the following will be done depending on your account status:
- Active phone customer – If you are a current phone customer then the phone rate for the primary line will be discounted.
- Non-active phone customer – If you do not currently have active phone service with Cox, please order via one of the following methods:
- Sign up for service online.
- Visit your closest Cox Store location.
- Call us at (402) 575-5276.
NOTE: Your Lifeline Telephone Application cannot be approved until:
- All questions / sections have been filled out on both sides of the form
- You signed the application
- Provided your date of birth
- Provided Social Security Number (Tribal ID if no Social Security Number)
- A copy showing proof of participation / eligibility is included with the application
Once enrolled, Lifeline customers will be contacted on an annual basis to re-certify their continued eligibility to receive the Lifeline discount.
Eligibility
You may be eligible for Cox's Lifeline Credit Program if you are a participant in any of the qualifying programs listed below, or if you qualify under the current federally established income based guidelines.
- Supplemental Nutrition Assistance Program (SNAP)
- Supplemental Security Income (SSI)
- Federal Public Housing (Section 8 Housing)
- Medicaid
- Low Income Home Energy Assistance Program (LIHEAP)
- Temporary Assistance for Needy Families (TANF)
- National School Lunch Program (NSLP)
- Kids Connection, SAM, MAC, EMAC
Once enrolled, Lifeline customers will be contacted on an annual basis to re-certify their continued eligibility to receive the Lifeline discount.
NOTE: Cox Phone service must be in the name of the qualifying Lifeline applicant.
