Oklahoma Lifeline Telephone Program
Lifeline local phone service is available to low-income consumers who qualify.
Steps to Enroll
To begin receiving Lifeline program discounts, you will need to submit a Lifeline Telephone Application (English or Spanish) and proof of participation / eligibility requirements to Cox (please refer to section 6 of the application). You may drop off the completed form and proof of participation / eligibility at a Cox Store location near you, fax it to (405) 600-9255 or mail it to the address below:

Once Cox has received these, one of the following will be done depending on your account status:
- Active phone customer – If you are a current phone customer then the phone rate for primary line will be discounted to $1.
- Non-active phone customer – If you do not currently have active phone service with Cox, please order via one of the following methods:
- Sign up for service online.
- Visit your closest Cox Store location.
- Call us at (405) 600-8282.
NOTE: Your Lifeline Telephone Application cannot be completed until:
- All questions / sections have been filled out on both sides of the form
- You signed the application
- Provided your date of birth
- Provided Social Security Number (Tribal ID if no Social Security Number)
- A copy showing proof of participation / eligibility is included with the application (see section 6 of the application)
Once enrolled, Lifeline customers will be contacted on an annual basis to re-certify their continued eligibility to receive the Lifeline discount.

Eligibility
Eligible consumers receive Enhanced Lifeline for $1 a month.
You are eligible for the Enhanced Lifeline Telephone Service program if you live on Tribal land (all Oklahoma counties are considered tribal land, except Beaver, parts of Beckham, Cimarron, Greer, Harmon, Jackson and Texas counties) and are a participant in any of the qualifying government programs OR qualify within the federally established annual income-based guidelines.
Once enrolled, Lifeline customers will be contacted on an annual basis to re-certify their continued eligibility to receive the Lifeline discount.
NOTE: Cox Phone service must be in the name of the qualifying Lifeline applicant.

- Tribally-Administered Temporary Assistance for Needy Families (TTANF)
- Head Start Programs (under income qualifying eligibility provision only)
- The National School Lunch Program's Free Lunch Program (NSLP)
- Medical Assistance (Medicaid)*
- Supplemental Nutrition Assistance Program (SNAP - formerly known as Food Stamps)
- Supplemental Security Income (SSI)
- Federal Public Housing Assistance (Section 8)
- Low-Income Home Energy Assistance Program (LIHEAP)
- Temporary Assistance to Needy Families (TANF)
- Vocational Rehabilitation (including hearing impaired)
- Oklahoma Sales Tax Relief*
- Food Distribution Program on Indian Reservations (FDPIR)
* Medical Assistance (Medicaid) includes SoonerCare – Oklahoma’s state health care provided to children under the age of 19, adults with children under the age of 18, pregnant women and people who are older than 65 or have blindness or another disability. The following programs are a part of SoonerCare: Aged, Blind and Disabled; Family Planning; Health Care for Children with Disabilities (TEFRA); and Health Screening, Diagnosis and Treatment (EPSDT).
