Louisiana Lifeline Telephone Program
Lifeline local phone service is available to low-income consumers who qualify.
Steps to Enroll
To begin receiving Lifeline program discounts, you will need to submit a Lifeline Telephone Application and proof of participation / eligibility requirements to Cox (please refer to step 2 of the application). You may drop off the completed form and proof of participation / eligibility at a Cox Store location near you, fax it to (877) 873-7188 or mail it to the address below:

Once Cox has received these, one of the following will be done depending on your account status:
- Active phone customer – If you are a current phone customer then the phone rate for primary line will be discounted by $14.00 a month, which consists $7.00 off your phone service and a waiver of the $7.00 FCC Access charge.
- New phone customer – If you do not currently have active phone service with Cox, please order via one of the following methods:
- Sign up for service online and submit your Lifeline application as per the instructions below.
- Visit your closest Cox Store location.
- Call us at 504-304-8444.

NOTE: Your Lifeline Application cannot be approved until:
- All questions / sections have been filled out on both sides of the form
- You have signed the application
- You have provided your date of birth
- You have provided your Social Security Number (last 4 digits only)
- You have provided a copy of official documentation showing proof of participation / eligibility with your application (see step 2 of the application)
Once enrolled, Lifeline customers will be contacted on an annual basis to re-certify their continued eligibility to receive the Lifeline discount.

Eligibility
You are eligible for the Lifeline Telephone Service program if you participate in any of the qualifying government programs OR qualify within the federally established annual income-based guidelines.
Once enrolled, Lifeline customers will be contacted on an annual basis to re-certify their continued eligibility to receive the Lifeline discount.
NOTE: Cox Phone service must be in the name of the qualifying Lifeline applicant.
Program Based Eligibility:
- Temporary Assistance for Needy Families (TANF)
- The National School Lunch Program's Free Lunch Program (NSLP)
- Medicaid
- Supplemental Nutrition Assistance Program (SNAP - formerly known as Food Stamps)
- Supplemental Security Income (SSI)
- Federal Public Housing Assistance (Section 8)
- Low-Income Home Energy Assistance Program (LIHEAP)
