Universal Lifeline Telephone Service (ULTS)
Enhanced Lifeline local phone service is available to low-income consumers who qualify.
Program Overview
The Universal Lifeline Telephone Service plan, better known as the Lifeline program, assists low-income applicants by reducing their monthly costs for one telephone line, per household, at the principle place of residence.
You may be eligible for the $7 per month credit on your telephone service and installation discounts if you receive assistance from qualifying government programs or meet annual income guidelines.
NOTE: Phone service must be in the name of the qualifying applicant.
Eligibility
You are eligible for the Universal Lifeline Telephone Service program if you are a participant in any of the qualifying government programs listed on the Lifeline application, or if you qualify under the current federally established annual income-based guidelines.
Once enrolled, Lifeline customers will be contacted on an annual basis to recertify their participation in a qualifying program.
How to Apply for the Lifeline Credit
We will need a signed self-certification form, available in both English and Spanish, to enroll you in Lifeline service. Print, complete and mail the form with the required supporting documentation to:
